Preparation for Adulthood and Annual Reviews

We would like to hear about your recent Education, Health and Care Plan (EHCP) annual review. The area that we want to hear about is called 'preparation for adulthood' and it covers these four areas:
- Education, employment and training
- Independent living
- Friends, relationships and community
- Health
These should be discussed at all ages and stages.
