The Learning Disabilities Partnership Board in Sheffield is looking for new reps to join the board.

Rep is short for representative. It means somebody who is speaking for a group of people.

How to be a rep

Read the role description at the bottom of this page to find out what it involves.

If you want to be a rep:

  • fill in the application form at the bottom of this page
  • complete the equality monitoring form
  • find a photo or picture of yourself.

Help with the form

To help with the questions you can:

  • chat with someone about why you want to be a rep
  • get help with the form from family, a friend or a support worker.
  • get help from someone from our team at the disability week events.

Send in your application

Send your application to Fiona Crowe at the Council:

Different formats

Instead of filling in the application form, you could make a video or a voice recording.

You can also ask Fiona if you need this form in a different format like Makaton or another language.

Deadline

You must send in your form by Tuesday the 27th June. If you are applying by post please contact fiona.crowe@sheffield.gov.uk who can provide you with a pre-paid return envelope. Please make sure that if you want to apply by post, you send it no later than the 19th June, to make sure we get it in time.

We will send a letter to everyone in Sheffield who has a learning disability to tell them who their new reps are.
